COMMUNI-T is a care collective of committed community support and healthcare workers partnered together to create lasting impact. Our team includes:

  • Doulas – Compassionate support throughout pregnancy, birth and postpartum
  • Certified Nurse-Midwives – Comprehensive women’s health care throughout the lifespan
  • Community Health Workers – Advocacy and access to social and healthcare services
  • Enhanced Care Management (ECM) – Individualized support for at-risk populations
  • Peer Support Specialists – Support during recovery and overall well-being
  • Community Housing Developers – Create safe, sustainable living spaces

Our Initiatives

Birth Equity & Infant Health Mobile Clinics

We are proud to introduce our state-of-the-art mobile clinics designed to meet the healthcare needs of underserved and rural communities. These clinics will travel across the Sacramento/Bay Area, Northern Rural Counties, and Central Valley Counties, ensuring that families in these areas have equitable access to quality care and support.

The Doula Help Desk

Supporting birth workers and maternal health organizations is at the heart of our mission. Through the Doula Help Desk, we provide technical assistance and resources to strengthen birth equity. We are focused on creating a supportive environment where doulas can provide culturally responsive care to expectant mothers and families. CAL AIM – ECM Community Supports (CS)

Through CAL AIM (California Advancing and Innovating Medi-Cal), we deliver Enhanced Care Management (ECM) and Community Support Services (CS) to improve health outcomes for vulnerable populations.

Community Housing Development

We understand that stable housing is a cornerstone of health and wellbeing. Our initiative focuses on building affordable and sustainable housing for those who need it most. By creating safe, supportive, and accessible homes, we are helping families establish a foundation for long-term health and economic stability.
